Add form into product.yaml

Hi all.I have a problem with product.yaml seting. The final point is to create a product page via admin panel with contact form inside. I need to create form automatically instead of writing it everytime into header product.md file.Is there any way to include form into product.yaml or some the best solution? Thanks before for help.

I use a hidden page to store form's settings in its header and build a new plugin to insert the form to the pages which requires the form. Please see https://getgrav.org/forum#!/general:global-form
