One form, multiple standard pages

Hi, I'm unclear how to use one form on multiple pages. The documentation describes using forms in modular pages but I just want to use one form on multiple basic or "standard" pages. Do I have to put the form definition in each page markdown file? This doesn't seem right. Please advise. Thanks.
